Bryant Intramurals Signup Form
Please fill out the signup form with as much information as possible.  Forms should be submitted BEFORE the captain's meeting. 
Each Team Must Pay at the Captain's meeting, or BEFORE the first games.  If special arrangements are made, you can pay the first week.
If signing up for softball, and you have more people than slots to enter names, don't worry about it, it isn't that important

IF SIGNING UP FOR SOCCER OR VOLLEYBALL, PLEASE WRITE THE WORD "SOCCER" or "VBALL" AFTER YOUR TEAM NAME.  THERE IS A PROBLEM WITH THE FORM, IF THIS IS NOT DONE, I WILL NOT KNOW WHAT SPORT YOUR TEAM IS FOR
